FARMINGDALE — Investigators from the National Transportation Safety Board have released a preliminary report into what caused a World War II-era aircraft to spiral out of the sky and crash into a Melville neighborhood on May 30.

The crash killed the pilot and sole occupant, 52-year-old Ken Johansen. Johansen and the aircraft had participated in the Bethpage Airshow at Jones Beach just days before the deadly crash.

In addition, the report says that the crash happened short after take-off, and that it appeared that no recovery had been initiated.
