The community of Scotch Plains is mourning the death of a 17-year-old resident who was fatally injured in a head-on crash. Police are asking for the public's assistance as they investigate what happened.

Dramatic video shows the moment Dan Triano was airlifted to a hospital after he was involved in a head-on collision along Cooper Road Wednesday afternoon.

Scotch Plains police say Triano, a junior at the Academy of Information Technology, was driving east on Cooper Road in his Ford Focus and somehow collided with a Ford Edge driven by a 45-year-old Scotch Plains woman.

The unidentified woman was taken to the hospital with non-life threatening injuries. Triano was airlifted to a hospital from Ponderosa Park, located a short walk away, but later passed away during surgery.

Flowers and candles have been set up at a makeshift memorial at the scene of the accident.

Many students took to Twitter to mourn the loss of the student-athlete by using the hashtag #RIPdan, wherein they shared posts about the talented hockey player.

Police are now asking anyone with additional information about the crash to contact them, as the incident still remains under investigation.
